Payment
We ask that as a patient you pay-as-you-go. We accept cash, check, credit card(Visa \ Master Card \ Discover \ American Express)and CareCredit. Minimum payments are required as follows:
- One third at onset of care
- One third midway
- One third upon completion
- Other arrangements must be agreed upon in advance
Insurance
Our office accepts assignment of benefits for many insurance companies. This allows us to file your insurance claims as a courtesy to you, and allows the insurance company to remit payment directly our office. However, not all insurance companies will accept such assignments of benefits. In these cases, you will be required to make payment in full for services as services are rendered and we will still file the claims on your behalf. Our office can only provide insurance estimates and your actual indemnity may be less. Your insurance coverage is an agreement between you and your employer. We will do our best to submit the appropriate forms as directed; however, it is your responsibility to ensure their payment and to remit payment for charges not covered under your plan.
Missed appointment
Please help us to serve all our patients by keeping your appointment. Every time an appointment is made a certain amount of time is reserved for you. If you cancel with less than 24 hours notice, we do not have the opportunity to offer your appointment time to someone else. Therefore, except for unforeseen circumstances, there will be a charge of forty dollars if appropriate notice is not given. In some cases a payment may be required prior to scheduling your next visit.
