Initial Appointment and Forms:
Your initial appointment will consist of Dr. Bernier listening to your concerns, reviewing your medical and dental history, followed by a complete dental examination, including any necessary x-rays. In some cases a second appointment may need to be scheduled to complete the consultation. In this way Dr. Bernier can carefully analyze your individual case between your visits and be prepared to thoroughly explain your diagnosis and any possible treatment options to you. The fee for the initial appointment is $86 and is due at the time of your visit.
We ask that you assist us by providing the following information at the time you schedule your initial examination:
- Reason for your appointment
- Who we can thank for the kind referral
- Recent X-rays we could use
- Dental insurance we may assist you in processing
